- Crystallized and Fluid Cognition in Adults Who Stutter
Purpose: There is a long-standing perception that individuals who stutter are less intelligent, with the disfluencies unique to stuttered speech often assumed to be the overt reflection of lower intelligence, despite no supporting evidence. The purpose of the present study was to explore the validity of this assumption by examining the cognitive abilities of adults who stutter compared to the general population using the NIH Toolbox (C) Cognition Battery (NIHTB-CB). Method: Sixty-three adults who stutter completed the NIHTB-CB, which includes seven standardized measures assessing crystallized cognition (Picture Vocabulary, Oral Reading) and fluid cognition (List Sorting Memory Test, Pattern Comparison Processing Speed Test, Flanker Inhibitory Control Test, Dimensional Change Card Sort Test, Picture Sequence Memory Test). The NIHTB-CB generates t-scores adjusted for demographic variables based on a large sample of neurotypical adults. Results: Composite scores of overall cognition for adults who stutter were not statistically equivalent, rather, their scores were higher than the general population. Higher scores were driven by crystallized cognition, with significantly higher scores on Oral Reading subscale. Conclusions: Present findings demonstrate that adults who stutter possess cognitive skills that are comparable to or potentially higher, than the general population. These results challenge the misconception that stuttering reflects diminished intelligence and offer evidence to mitigate stereotype threat.
- Projected burden of alcohol-associated liver disease in China, 2020-2050: A microsimulation modeling study
Background Alcohol-associated liver disease (ALD) has emerged as a major cause of chronic liver disease and liver-related mortality in China. This study aimed to project the future burden of ALD in Chinese adults from 2020 to 2050, including prevalence of ALD, number of alcoholic steatohepatitis (ASH) cases, incident hepatocellular carcinoma (HCC) cases, liver transplantation (LT) demand, liver-related deaths, and disability-adjusted life years (DALYs). Methods We developed an agent-based state-transition microsimulation model with yearly cycles and a lifetime horizon. The model simulated 5,678,912 representative Chinese adults (mean age 36.2 years, 51.2% male). Health states included no steatosis, alcohol-associated steatotic liver, ASH, fibrosis stages F0-F4, decompensated cirrhosis, HCC, LT, and liver-related death. Model inputs were derived from the China Kadoorie Biobank, Global Burden of Disease Study 2021, China's national surveys, published meta-analyses, and transplant registry data. Projections incorporated demographic shifts, alcohol consumption trends, and calibrated transition probabilities. Uncertainty was assessed via 1,000 Monte Carlo simulations generating 95% uncertainty intervals. Results ALD prevalence was projected to increase from 4.8% (55 million individuals) in 2020 to 8.5% (94 million individuals) by 2050. ASH cases rose from approximately 18 million to 20 million. Annual incident HCC cases nearly doubled from 20,500 in 2020-2025 to 45,200 by 2046-2050. LT demand quadrupled from 2,300 to 9,800 cases. Liver-related deaths increased from 50,000 in 2020 to 85,000 in 2050, while DALYs rose from 1.5 million to 2.6 million. Conclusions In the absence of strengthened alcohol control policies, ALD will impose a substantial and growing burden on China's health system by 2050, with marked increases in HCC incidence, LT demand, and liver-related mortality.
- Regional endemicity of toxigenic Vibrio parahaemolyticus lineages associated with foodborne illness in Australia
Gastrointestinal Vibrio parahaemolyticus infections, primarily associated with consumption of oysters, are emerging in Australia, where previously little was known about the disease and epidemiology. Following a multijurisdictional outbreak in 2021 and additional smaller outbreaks in subsequent years, an opportunistic whole genome sequencing study was undertaken to characterise human illness-causing strains in Australia. Through a multijurisdictional collaboration that bridged research, government, pathology service providers, aquaculture and clinicians, 676 V. parahaemolyticus genomes were contributed for analysis from human clinical, food, and environmental samples. We identified ST36, ST50 and ST417 as the dominant multi-locus sequence types causing gastrointestinal illness nationally. Phylogeographic contextualisation of Australian V. parahaemolyticus sequences within the global dataset indicates the Australian and New Zealand ST36 strain originated from a single point of introduction from the US Pacific-Northwest and is now circulating locally. In contrast, ST50 and ST417 appear to be endemic across Australia, with multiple lineages co-circulating. These findings establish a baseline for future outbreak investigations of V. parahaemolyticus in Australia and the consolidation of Australian data provides a critical platform for ongoing research, public health surveillance and risk mitigation.
- Psychometric Properties of the AASPIRE Autistic Burnout Measure - Revised (AABM-R)
Background: Autistic Chronic Energy Depletion Syndrome, commonly referred to as Autistic Burnout, is a debilitating condition characterized by exhaustion, loss of function, and reduced tolerance to stimuli. While several instruments attempt to measure it, validation studies have only used cross-sectional designs and/or convenience samples with low support needs, limiting understanding of their performance across heterogeneous, autistic populations and over time. Methods: Using a community-based participatory research (CBPR) approach, we revised the 27-item AASPIRE Autistic Burnout Measure (AABM) into a 14-item AASPIRE Autistic Burnout Measure-Revised (AABM-R) and tested it in a longitudinal study of 835 autistic adults recruited from healthcare systems, disability services, and the community. Participants completed surveys directly (with or without support) or via a caregiver. We assessed structural validity and measurement invariance using exploratory and confirmatory factor analysis, tested construct validity through a priori hypothesis testing, examined discriminant validity from depression using longitudinal factor analysis and cross-lagged panel models, and assessed criterion validity using ROC analysis. Results: The AABM-R demonstrated a clear single-factor structure among direct reporters, with and without support, and measurement invariance across these groups; findings were less conclusive for the smaller caregiver-report subsample. Autistic burnout correlated as hypothesized with stressors (e.g., discrimination, masking, adverse childhood experiences), supports (e.g., social support, receiving needed help with daily living activities), and broader outcomes (e.g., quality of life, depression, anxiety). Longitudinal modeling supported autistic burnout as empirically distinct from, though related to, depression. ROC analysis (AUC = 0.89) supported cut-offs distinguishing probable (33-56, LR 7.38), unsure, and unlikely (0-22, LR 0.15) burnout. Conclusions: The AABM-R is a brief, accessible, psychometrically sound measure of autistic burnout suitable for heterogeneous autistic populations, with preliminary clinical cut-offs to guide screening. Further research is needed on the caregiver-report version and on longitudinal predictors and outcomes of burnout.
- Mental health contact gaps among adults in Peru: a cross-sectional analysis of a nationally representative survey
Objective. To estimate lifetime and 12 month mental health contact gaps among Peruvian adults with survey-defined mental disorders, and to describe inequalities in contact, perceived need for care, and mental health service use. Methods. We analyzed the information for adults of the 2022 Peruvian National Mental Health Survey, a cross-sectional household survey. The primary outcome was the survey-weighted proportion of adults with a 12 month mental disorder who reported no contact with an included source of mental health-related care during that period; the lifetime contact gap was descriptive. Perceived need was assessed using two derived 12 month perceived-need measures based on direct ENSM variables and service contact routing items. Analyses incorporated weights, strata, and clusters. Adjusted prevalence ratios were estimated using survey weighted Poisson models. Results. The dataset contained information on 13,840 individuals; 13,833 had complete survey-design information. Contact gap denominators were 3,927 for lifetime disorders and 1,649 for 12-month disorders. The lifetime and 12-month contact gaps were 61.0% (95% CI 58.4-63.7) and 84.5% (95% CI 81.5-87.6), respectively. Rural estimates exceeded urban estimates in both periods; after adjustment, poverty and rural residence were associated with the lifetime gap, and extreme poverty with the 12-month gap. Among individuals meeting survey-based criteria for one or more 12-month mental disorders, 37.3% (95% CI 33.4-41.4) reported self-perceived need, whereas 25.6% (95% CI 21.8-29.8) reported that need had been identified by others. Annual psychological and psychiatric service use was 3.7% and 1.1%, respectively. Conclusions. Mental health contact gaps were high, particularly for one or more 12-month mental disorders, and were associated with social and territorial variables. These contact measures do not establish adequate, continuous, or effective treatment, which needs to be addressed to understand the impact of the Peruvian mental health reform.
- Predicting Anxiety Trajectories from Individual Differences in Sleep-Related Distress Alleviation
Importance Anxiety of fluctuating severity is common in many psychiatric disorders. Few studies addressed factors determining individual differences in trajectories of the recovery phase, while their identification could inspire treatment innovation. Given the role of REM sleep in overnight alleviation of emotional distress, we here investigate whether individual differences in this overnight regulatory process matter for anxiety recovery. Objective To investigate whether individual differences in overnight alleviation of distress by REM sleep predict anxiety recovery rate. Design People tend to volunteer for intervention trials when fluctuating symptoms peak. This results in a significant recovery even in waitlist or control conditions. Leveraging this opportunity to recruit people prior to the recovery phase, in this cohort study, we utilized data from people who volunteered for optional sleep EEG and overnight distress assessment prior to their participation in an intervention trial (2021-2025). Anxiety severity was assessed at baseline and two months later. Setting Home-based assessment in the Netherlands. Participants Adults with insomnia alongside cross-threshold symptom severities of generalized anxiety disorder, social anxiety disorder, panic disorder, posttraumatic stress disorder, or borderline personality disorder (N = 223, 157 female [70.4%]; mean [SD] age, 45.7 [14.5] years; clinical diagnoses confirmed in 165 [74.0%]). Exposures Cognitive behavioral therapy for insomnia (CBT-I) or waitlist control. Main Outcomes and Measures Predicting 2-month anxiety improvement by individual differences in the strength of the effect of REM sleep on overnight distress alleviation at baseline. Results Within-subject mixed model analysis showed stronger overnight distress alleviation across nights with longer REM sleep (b = -0.011; 95% CI, -0.016 to -0.007; P < .001). Individual differences in the strength of REM-related distress alleviation predicted anxiety improvement after two months (b = -0.521; 95% CI, -0.854 to -0.188; P = .002), irrespective of treatment or waitlist control (interaction b = 0.011; 95% CI, -0.656 to 0.678; P = .97). Conclusions and Relevance Individual differences in the degree to which REM sleep drives overnight alleviation of distress predict the trajectory of anxiety recovery in people with clinically relevant psychiatric complaints. These findings suggest REM-related emotion regulation as a mechanism linking sleep physiology to anxiety recovery.
- Developmental trajectories of suicidality and lifestyle factors across adolescent severity profiles
Background: Adolescent suicide is a leading cause of death in Australia, arising from multiple determinants. Psychological distress, lifestyle behaviours and socioeconomic factors are associated with adolescent suicidality. Existing research has predominantly employed cross-sectional designs, limiting our understanding of how these factors interact over time. Longitudinal and data-driven approaches are needed to help identify the factors associated with the emergence of suicidality throughout adolescence. Method: Participants aged 12-17 years completed longitudinal measures of suicidal ideation, psychological distress, sleep quality, mindfulness, physical activity, eating habits, and social connectedness. Subgroups were determined via hierarchical cluster analysis, based on average scores across later timepoints (9-15). ANOVA and pairwise effect size calculations were used to compare clusters across variables, and their preceding developmental trajectories were examined using generalised additive mixed models (across earlier timepoints; 1-8). Clusters were also compared on self-reported wellbeing, long-term suicidality, and socioeconomic status. Result: Three clusters characterised by low-, moderate-, and high-severity of suicidal ideation and psychological distress, and poorer sleep, social connectedness, physical activity, mindfulness, and eating habits were identified. Across earlier timepoints, the high-severity group showed consistently elevated suicidality and deteriorating wellbeing and lifestyle scores. Conclusion: Youth with high levels of suicidality had greater psychological distress, lower wellbeing, lower socioeconomic status, and poorer lifestyle behaviours. This subgroup was also found to have poorer scores on wellbeing and lifestyle factors in their early adolescence. Findings highlight the importance of early, preventative interventions targeting both mental health and lifestyle factors to reduce suicidality in adolescents.
- Distinct response of resting-state brain networks to psilocybin in autism
Importance There is increasing interest in the potential of psilocybin to treat mental health and neurodevelopmental conditions. At high doses, the therapeutic benefit of psilocybin is linked to greater functional connectivity or integration between large-scale brain networks which underpin mood, emotion and cognition. However, it is unknown how the brain responds to psilocybin in autism - a condition characterised by both altered functional connectivity and differential response to drugs. Thus, a first step before clinical trials of psilocybin involving autistic people, is to evaluate the response of the autistic brain to psilocybin, initially at low dose. Objective Low doses of psilocybin were used to test the hypothesis that the functional connectivity of large-scale resting-state brain networks respond differently in autistic and non-autistic adults. Design The PSILAUT study had a pseudo-randomised, cross-over, double-blind, case-control design. There was no evaluation of clinical efficacy. PSILAUT was not a Clinical Trial according to UK regulations. Data collection was conducted from January 2023 to August 2024. Setting Single-centre, study conducted at the Institute of Psychiatry, Psychology & Neuroscience, Kings College London, London, United Kingdom. Participants Adult (> 18 years) participants with and without an autism spectrum disorder (ASD) diagnosis were recruited and matched for age, sex and IQ. Autistic participants were included if they had an existing diagnosis (DSM-IV, DSM-5 or ICD-10 criteria). Exposures A single oral dose of 2 or 5 mg psilocybin or (inactive) placebo administered on separate visits at least one week apart. Main Outcomes and Measures Resting-state fMRI was acquired to investigate the change in functional connectivity within and between brain networks, as measures of network integrity and integration, respectively. Results A total of 67 participants were recruited (18-58 years at first visit; 30 non-autistic participants, mean [SD] age, 30.0 [8.3] years, 15 males [50%] and 37 autistic participants, mean [SD] age, 28.6 [9.2] years, 19 males [51%]). We report for the first time that the autistic brain responds differently to low doses of psilocybin, despite no group differences in network connectivity in the baseline placebo condition. 5 mg psilocybin elicited the greatest shifts in functional connectivity in both groups, but in different directions. In non-autistic participants only, on average, after 5 mg psilocybin within-network connectivity of the frontoparietal ({beta} = -0.053, T = -2.73, FDR-corrected P value = 0.027, Cohen d = -0.71) and limbic networks ({beta} = -0.087, T = -2.59, FDR-corrected P value = 0.021, Cohen d = -0.61) decreased. In contrast, in autistic participants, 5 mg psilocybin increased between-network connectivity (i.e. integration) of higher-order and attentional networks, but decreased connectivity between the same networks in non-autistic participants (default mode and frontoparietal networks, dose x group interaction: {beta} = 0.053, T = 2.91, FDR-corrected P value = 0.042; dorsal and ventral attention networks, dose x group interaction: {beta} = 0.059, T = 2.31, FDR-corrected P value = 0.015). Across the whole sample, the extent to which psilocybin elicited an increase in connectivity between higher-order ({beta} = 0.33, T = 2.16, FDR-corrected P value = 0.036) and attentional ({beta} = 0.36, T = 2.43, FDR-corrected P value = 0.036) networks was positively correlated with core autistic traits quantified using the Autism Quotient. Conclusions and Relevance Functional brain networks that support mood, emotion and cognition are more responsive to low dose psilocybin in autistic adults compared to non-autistic adults. Given that increased network integration is associated with clinical utility, future applications of psilocybin in autistic people should include the evaluation of low doses.
- Sub-national heterogeneity in the time-varying reproduction number during the 2026 Bundibugyo virus disease outbreak in the Democratic Republic of the Congo: a hierarchical Bayesian analysis
National-level estimates of the time-varying reproduction number (Rt) for the 2026 Bundibugyo virus disease (BDBV) outbreak in the Democratic Republic of the Congo (DRC) have converged on a value close to the epidemic threshold since early August 2026, consistent with independent joint Bayesian renewal-model estimates. A single national Rt, however, can obscure divergent sub-national epidemic trajectories, particularly across a five-province outbreak in which provinces range from a declining original epicentre to recently-seeded fronts. We estimated Rt at national, provincial, and, where case volume allowed, health-zone level, using both a standard sliding-window (Cori) estimator and a hierarchical Bayesian renewal model with partial pooling across spatial units, fitted by Hamiltonian Monte Carlo (No-U-Turn Sampler). Provincial estimates diverged materially from the national trend: as of the week of 6 August 2026, Ituri, the outbreak's original epicentre, had a hierarchical median Rt of 0.91 (95% credible interval [CrI] 0.68 - 1.24), while Nord-Kivu (1.23, [0.87 - 1.72]) and Haut-Uele (1.79, [1.24 - 2.67]) remained above threshold. Health-zone disaggregation, feasible only in Ituri and Nord-Kivu given case volume, showed this provincial picture itself masked further heterogeneity: in Ituri, the zone where the outbreak began (Mongbwalu) had clearly declined (Rt 0.36, [0.17 - 0.74]) while the two largest zones by cumulative case count (Bunia, Rwampara) remained at or above threshold; in Nord-Kivu, elevated transmission was concentrated in a single zone (Katwa, Rt 1.39, [0.85 - 1.99]) while a comparably-sized zone (Butembo) had already declined (0.64, [0.23 - 1.38]). An initial disagreement between the sliding-window and hierarchical provincial estimates was traced to a data-reconstruction artefact (forward-filling, rather than interpolating, multi-day gaps in health-zone reporting) rather than a genuine methods disagreement, and resolved once corrected. The hierarchical model's dispersion structure, calibration, and sensitivity to the generation-interval assumption were each checked explicitly; a shared (non-province-specific) dispersion parameter was retained on the basis of negligible predictive difference (PSIS-LOO), the model achieved 95.0% pooled 95% posterior-predictive interval coverage, and the province ranking was unchanged across a generation-interval sensitivity grid (Spearman {rho} = 1.0). Aggregation masks meaningful heterogeneity in transmission intensity at every spatial resolution examined; response prioritisation based on a single national or even provincial Rt risks directing attention away from the specific zones where transmission remains supercritical.
- Rethinking respiratory disease forecasting: temporal heterogeneity between surveillance predictors and outcomes drives forecast instability
Since the COVID-19 pandemic, forecasting hubs and non-traditional respiratory disease surveillance streams have become increasingly common. However, many forecasting approaches assume that relationships between surveillance predictors and disease outcomes remain stable over time and that incorporating additional historical data will improve forecast performance. To evaluate these assumptions in a real-world setting, we developed and evaluated forecasts of SARS-CoV-2 and influenza hospitalizations in Utah using syndromic surveillance, test positivity, and wastewater data. Rather than identifying a single, best-performing model, we examined whether relationships between surveillance predictors and hospitalization outcomes remained stable across seasons and whether longer historical training periods consistently improved forecast accuracy. Relationships between surveillance predictors and hospitalizations varied substantially by pathogen and season. Analyses using pooled data across multiple years suggested strong positive correlations between predictors and outcomes, but these aggregated patterns often obscured weak or negative correlations observed during SARS-CoV-2 variant waves and influenza seasons. Forecast performance similarly varied over time. Models that performed well during some seasons, transmission phases, or under certain training strategies frequently performed worse than benchmark models in others. Training on additional historical data generally reduced forecast accuracy, though this varied by disease and transmission phase. Forecasting groups should prioritize continual evaluation of surveillance predictors, adaptive strategies, and diverse ensembles, rather than relying on a single model, data stream, or historical training framework each year.
- Evaluation of Sex Hormones in Autoimmune Diseases in Pre- and Post-Menopausal Women
Autoimmune diseases have shown biased proportion in female population, existing clinical investigations of sex hormones in autoimmune populations have been relatively limited in terms of patient size and types of hormones investigated. In this study, we examined the relationship of sexual hormones and autoimmune antibodies in a large cohort of US women. This retrospective study sample included a total of 15319 female subjects medical information that were collected between December 2015 to May 2019 and tested in the Vibrant America Clinical Laboratory. The present serum sample was limited to female participants who had ever menstruated at the time of blood collection and completed the testing of the autoimmune antibodies and sex hormones. We focused on a total of 13 clinically significant autoantibodies including antinuclear antibody (ANA), 11 anti-extractable nuclear antigens (anti-ENAs), anti-cyclic citrullinated peptide 3 (anti-CCP3), and 11 female sex hormones. First, the prevalence of serological autoantibodies in a large set of adult female subjects divided by the menopause age was investigated. Next, the levels of sex hormones were compared in the seropositive autoimmune subjects and seronegative controls across the pre- and post-menopausal female groups. The presented study involving a large cohort of females showed no statistically different levels of sex hormones in seropositive autoimmune subjects and matched controls except for DHEA-s.
- Cue-Induced Retrieval and Reconsolidation with Episodic Future Thinking for Craving and Delay Discounting in Opioid Use Disorder: A Pilot Randomized Controlled Trial
Aims: To evaluate whether Cue-Induced Retrieval and Reconsolidation with Episodic Future Thinking (CIREF), which combines personalized drug-cue retrieval with structured future-oriented processing, produces greater changes in craving and delay discounting than a recent-past episodic active control in individuals with opioid use disorder receiving methadone maintenance treatment. Design: Multicentre, two-arm, parallel-group randomized controlled pilot trial with per-protocol analyses. Setting: Two outpatient addiction treatment and rehabilitation centres in Tehran, Iran. Participants: Thirty participants with opioid use disorder receiving methadone maintenance treatment were randomized to CIREF or Episodic Recent Thinking (ERT; n = 15 per group). Twenty-eight completed the intervention and were included in the analyses (n = 14 per group). Intervention and comparator: Participants completed one screening, baseline, and personalized cue-development session followed by three 75-minute intervention sessions. CIREF combined personalized drug-cue retrieval with future-oriented simulation, prediction, intention, and planning. ERT was structurally matched but anchored episodic processing to the recent past. Measurements: Primary craving outcomes comprised the three Desire for Drug Questionnaire (DDQ) subscales assessing session-level phasic/current craving immediately before and after each intervention session and the four Obsessive-Compulsive Drug Use Scale (OCDUS) subscales assessing tonic craving before and after the intervention period. The secondary outcome was Monetary Choice Questionnaire (MCQ) log(k), with more negative values indicating less steep delay discounting. Findings: After Holm correction across the three DDQ subscales, Group x Occasion interactions indicated greater reductions with CIREF for Desire and Intention to Drug Use, FGG(1.23, 32.09) = 24.37, pHolm < .001, partial eta-squared = .484, and Negative Reinforcement, FGG(1.35, 35.23) = 17.67, pHolm < .001, partial eta-squared = .405, but not Drug Abuse Control (pHolm = .172). After Holm correction across the four OCDUS subscales, only Desire and Mental Preoccupation with Drugs showed a significant Group x Time interaction, F(1, 26) = 12.35, pHolm = .007, partial eta-squared = .322; the remaining subscales were not significant (adjusted ps >= .177). MCQ log(k) showed a Group x Time interaction, F(1, 26) = 7.18, p = .013, partial eta-squared = .217; mean log(k) changed from -1.22 (0.36) to -1.80 (0.55) in CIREF and from -1.39 (0.32) to -1.44 (0.44) in ERT. Conclusions: In this small pilot sample, the future-oriented retrieval-based intervention produced greater changes than the recent-past active control in two dimensions of session-level phasic craving, one dimension of tonic craving, and monetary delay discounting. The results are preliminary and do not establish memory reconsolidation or effects on relapse or longer-term clinical outcomes.
- Effect of transitioning virally suppressed children and adolescents with HIV to dolutegravir-based antiretroviral therapy: emulated target trials in a large cohort in South Africa
Background: Global HIV programmes are transitioning virally suppressed children and adolescents with HIV (CAWH) from prior regimens to dolutegravir-based antiretroviral therapy (ART). However, the supporting evidence largely stems from randomised trials in viraemic CAWH. The effect of transition for virally suppressed CAWH is unknown. Methods: We used observational, de-identified data from 724 clinics in KwaZulu-Natal, South Africa. We sequentially emulated three distinct target trials to estimate the effect of transitioning to dolutegravir-based ART in three paediatric populations: i) ages 8-17 years taking efavirenz-based ART, ii) 8-17 years taking ritonavir-boosted lopinavir (LPV/r)-based ART, and iii) 0-7 years taking LPV/r-based ART, all with a last viral load <1,000 copies/mL. The risk difference (RD) of death or viraemia >1,000 copies/mL through 12 and 24 months was estimated using an inverse probability weighting approach. Findings: From January 2020 to August 2024, 37,145 CAWH contributed 454,081 person-trials. In CAWH initially taking efavirenz, the standardised 12-month risk of death or viraemia was 11.9% with continued efavirenz and 6.7% with transition to dolutegravir (RD -5.2 [95% CI -5.8 to -4.6]). In older CAWH initially taking LPV/r, these risks were 17.8% and 9.5%, respectively (RD -8.3 [-10.0 to -6.8]). In younger children, the respective risks were 15.8% and 6.7% (RD -9.0% [-12.7 to -5.4]). Where available, 24-month endpoints showed slightly greater RDs. - Regulation-Driven Variation in Utilization and Cost of B-Cell Depleting Therapies for Multiple Sclerosis: A Cross-National Study
Background: Rituximab and ocrelizumab target the same CD20 receptor. Rituximab is off-patent and prescribed off-label; ocrelizumab is licensed and patent-protected. Whether the resulting differences in utilization and cost reflect clinical value or regulatory structure has not been examined. Objective: To determine whether utilization and cost of B-cell depleting therapy across six health systems track regulatory approval status more closely than comparative effectiveness. Methods: We examined rituximab and ocrelizumab utilization and cost in Sweden, France, Germany, the United Kingdom, Italy and the United States (2016-2024). Costs were drawn from published national sources on a consistent ex-factory basis. Utilization was registry-measured for Sweden, France and Germany, measured from national claims for the United States, and estimated from indirect data for the United Kingdom and Italy. Weighted annual costs per patient on B-cell depleting therapy were modeled by Monte Carlo simulation (10,000 iterations). Results: Rituximab constituted the near-totality of B-cell depleting therapy in Sweden but 2.3% to 18.7% of use in the other five systems. Mean annual cost per patient ranged from $3,014 (Sweden) to $52,506 (United States), a 17-fold difference, with the four other European systems between $18,140 and $26,262. Adopting Sweden's utilization pattern was associated with modeled five-year per-patient differences in drug acquisition cost of $76,000 to $248,000. Conclusion: Utilization and cost align more closely with regulatory approval status than with available effectiveness data. International reference pricing acts on the price of the licensed agent but leaves intact the regulatory asymmetry that determines which agent is prescribed.

- Comparing HIV Testing Patterns Among Adolescent Girls, Young Women, and Older Women in Nigeria: A Comparative Analysis and Socioecological Correlates
Background HIV testing is a key entry point for prevention and treatment in Nigeria, yet age-related disparities persist. Adolescent girls and young women (AGYW) are disproportionately affected by HIV but often demonstrate lower uptake of testing services. This study examined differences in HIV self-testing awareness, use, and antenatal HIV testing between AGYW and older women, the distribution of HIV testing service uptake, and identified multilevel factors associated with HIV self-test awareness and testing among AGYW. Methods This study analyzed cross-sectional data from the 2023-2024 Nigeria Demographic and Health Survey (NDHS), including women aged 15-49 years. Descriptive statistics were used to assess differences in HIV self-testing awareness, use, antenatal HIV testing, and place of HIV testing between Adolescent Girls and Young Women (AGYW) (15-24 years) and older women (25-49 years). Among AGYW, bivariate and multivariate logistic regression were conducted to identify socioecological factors associated with HIV self-testing, awareness and use. Results A total of 14,708 AGYW and 24,342 older women were included in the analysis. The median age of AGYW was 19 years (IQR: 17-22), compared to 35 years (IQR: 25-41) among older women. Older women reported significantly higher awareness of HIV self-testing (13.1%) compared to AGYW (8.8%). Use of the HIV self-testing kit was low overall but higher among older women (2.3%) than among AGYW (1.1%). Older women reported greater uptake of antenatal HIV testing (57.9%) than AGYW (48.4%). Education, wealth, and internet use were positively associated with awareness, while AGYW aged 15-19 had significantly lower odds of awareness than AGYW women aged 20- 24. Regional disparities were also observed in the awareness and use of HIV self-test kits. Conclusions Findings support that targeted, AGYW-centred strategies are needed to improve equitable access to HIV testing and achieve national and global HIV prevention goals.
