Menopause affects over one billion women worldwide, yet remains poorly characterized at scale. We apply an ICD-10-based phenotyping algorithm to electronic health records (EHR) from an academic medical center (n=33,444 women aged 35-64) and a safety-net hospital system (n=7,041), yielding one of the most racially and socioeconomically diverse menopause cohorts in the literature. Structured EHR fields underrepresent symptom burden: only 38.8% of patients had any documented symptom via natural language processing, despite an estimated prevalence of 90%. Adverse pregnancy outcomes were associated with earlier menopause onset after adjustment (beta=-1.12 years, p=8.7x10^-45). Menopausal women showed elevated risk for osteoporosis (hazard ratio of 12.40), rheumatoid arthritis (HR of 2.43), and mental and behavioral disorders (HR of 2.38) relative to age-matched men, with divergence at menopause onset. INTRODUCTION: Educational attainment is a key proxy of cognitive reserve (CR), but whether its association with cognition persists after accounting for multimodal brain pathology remains unclear. METHODS: Data from PREVENT-AD and CIMA-Q were analyzed using cross-sectional, longitudinal, and residual-based models adjusted for multimodal pathology, including MRI-based white matter hyperintensities and atrophy measures, and plasma Alzheimer's disease biomarkers. Education was examined in both cohorts. A multidimensional CR questionnaire was also examined in CIMA-Q. RESULTS: Higher education was associated with better performance across multiple cognitive domains after accounting for pathology (p < 0.05). The CR composite score showed less consistent associations, and non-education subdomains showed limited added value. Longitudinally, education was more consistently associated with baseline cognitive levels than cognitive change, with executive-function trajectory differences observed in

Stimulation-induced motor contractions are among the most common dose-limiting side effects in subthalamic nucleus deep brain stimulation for Parkinson's disease, yet no quantitative models exist to predict their occurrence from imaging data. Here, we combine pathway activation modeling of the corticospinal and corticobulbar tracts within the posterior limb of the internal capsule with a data-driven prediction framework. Evaluated by leave-one-patient-out cross-validation across an intraoperative (42 patients, 352 sites) and postoperative sub-cohort (11 patients, 176 contacts), the model explained 31-35% of the variance in observed motor contraction thresholds. The model reliably identified contacts with the lowest and highest motor contraction thresholds within individual electrodes, with strongest performance for distinguishing directional segments at the same electrode level (84% and 64% accuracy; p < 0.001), supporting its potential value for postoperative DBS programming.
