Purpose: When emulating trials of medication initiation using real world data, there may be ambiguity regarding the most suitable time zero event for the research question of interest. The time zero event must be strongly associated with the clinical indication for treatment, confer a high probability of actual treatment initiation, and be measurable with sufficient temporal precision in the source data. When it is uncertain whether a candidate event will satisfy these three conditions simultaneously, empirical identification of predictors of medication initiation can provide valuable guidance. The objective of this study was to empirically identify predictors of incident atorvastatin initiation to inform the definition of time zero for future target trial emulations. Methods: A retrospective cohort study was conducted using Medicare claims data (study period: January 1, 2018 to December 31, 2019). - In vivo measurement and prediction of cell-type-resolved non-coding variant effects in human immunity
Interpreting non-coding variant effects in their native cellular context is the central problem in medical genomics. Using paired single-cell ATAC-seq and whole-genome sequencing in human immune cells, we measured allele-specific regulatory effects at 129,241 heterozygous variants across seven cell types. We trained REG-FOCUS, a transferable sequence-and-chromatin framework, and extended predictions to 1,188,417 common variants across twelve immune cell types. These predictions revealed that enhancer-centric variant directionality is governed by quantitative tuning of transcription factor binding affinity. Applied to systemic lupus erythematosus, REG-FOCUS prioritized causal regulatory variants selectively converging on pathways of approved therapies, and enabled us to identify rare enhancer variants in patients corroborated by expression outliers in carriers. This work brings cell-type-resolved variant interpretation to non-coding personal genomes.
- Effect of ancestry and shared genetic architecture of serious mental illness on symptoms and cognition in an admixed Latin American population
Most genome-wide association studies (GWAS) of serious mental illness (SMI) have been conducted for categorical diagnoses in samples of primarily European ancestry. The portability of findings to non-Europeans, and to SMI-related symptoms/dimensional traits remains uncertain. In a sample of 8,666 SMI cases and controls from the Paisa region of Colombia we show that a primarily European schizophrenia GWAS polygenic risk score (PRS) predicted all SMI diagnoses in this sample, as well as symptoms (assessed in cases only) and traits assessed agnostic to SMI diagnosis: a one SD unit (SDU) increase in this PRS was associated to decreased risk in cases of suicidal thoughts (OR=0.89, 95% confidence interval 0.84-0.94), depressed mood (OR=0.90, 95% confidence interval 0.85-0.95), and increased risk of delusions (OR=1.12, 95% confidence interval 1.06-1.18) and to decreased cognition (in cases and controls) across five distinct domains (average decrease in cognition of 0.065 SDU, p<7e-05).
